City Optix provides comprehensive optometric services including:

• Comprehensive eye examinations
• Contact lens fitting and dispensing
• Prescription eyewear (glasses and sunglasses)
• Designer and independent eyewear collections

• Eye health evaluations

All optometric services are provided by Dr. Jeff Rich, OD, a licensed optometrist in the State of California. Services are subject to applicable laws and regulations governing the practice of optometry.

4. ELIGIBILITY AND USER ACCOUNTS

4.1 Age Requirements
Our Website and Services are intended for users who are at least 18 years of age or the age of majority in their jurisdiction. If you are under 18, you may use our Services only with the involvement and consent of a parent or legal guardian.

4.2 Account Registration
To access certain features of our Services, you may be required to create an account. When creating an account, you agree to:

5.1 Appointment Scheduling
Appointments may be scheduled through our Website, by telephone at (415) 921-1444, or in person at our Marina District location. We reserve the right to confirm, reschedule, or cancel appointments at our discretion.

5.2 Cancellation Policy
If you need to cancel or reschedule an appointment, please notify us at least 24 hours in advance. Repeated missed appointments without notice may result in restrictions on future appointment scheduling.

5.3 Service Limitations

• Services are provided subject to Dr. Rich’s professional judgment and applicable laws
• We reserve the right to refuse service to any individual
• Certain services may require prescriptions, medical history, or insurance verification
• Eye examinations and medical services are subject to HIPAA privacy protections (see our separate HIPAA Notice of Privacy Practices)

6. PRODUCTS AND PURCHASES

6.1 Product Information
We strive to display accurate information about our eyewear products, including descriptions, pricing, and availability. However:

• We do not warrant that product descriptions, colors, or other content are accurate, complete, reliable, or error-free
• We reserve the right to correct errors, inaccuracies, or omissions at any time
• Product availability is subject to change without notice.

6.2 Pricing
All prices are in U.S. dollars and are subject to change without notice.

6.3 Payment:
We accept major credit cards, debit cards, FSA/HSA cards, and vision insurance. Payment is due at the time of purchase unless other arrangements are made.

6.4 Insurance:
We accept most major vision insurance plans including VSP, EyeMed, and MES. Insurance coverage and benefits are between you and your insurance provider.

7. PRESCRIPTIONS

7.1 Valid Prescriptions Required
A valid prescription from a licensed optometrist or ophthalmologist is required for prescription eyeglasses, contact lenses, and prescription sunglasses.

7.2 Prescription Verification
We reserve the right to verify prescriptions with the prescribing doctor before dispensing products.

8. RETURNS, EXCHANGES, AND WARRANTY

Non-prescription eyewear may be returned within 30 days of purchase with original receipt and in original condition. Custom or special-order items and opened contact lenses are non-returnable.

9. INTELLECTUAL PROPERTY RIGHTS

All content on our Website, including but not limited to text, graphics, logos, images, and designs, is the property of City Optix and protected by copyright laws.

10. USER CONDUCT

You agree not to violate any laws, infringe upon the rights of others, or interfere with the proper functioning of our Website.

11. HIPAA AND PROTECTED HEALTH INFORMATION

These Terms do not govern Protected Health Information (PHI). Our practices regarding PHI are described in our separate HIPAA Notice of Privacy Practices.

13. DISCLAIMER OF WARRANTIES

14. LIMITATION OF LIABILITY
